This logic model was prepared for the Office of Minority Health. The attached document is a logic model for a data intermediary that has moved past its startup phase. The model describes a data intermediary whose key activities include assembling, transforming, and maintaining data; disseminating information and applying the data to achieve impact; and using data to strengthen civic capacity and governance. The goals of a data intermediary depicted in this model include contributing to the reduction of inequalities in quality of life and public health, as well as empowering communities to advocate for their priorities.
