This is a reference and how-to guide for collecting, using, and disseminating data for community decisionmaking and information-sharing that may be used in conjunction with the Outcomes Toolkit (a software program created by Accelerating Community Transformation). It describes resources and methods available for implementing an outcome-based approach to developing healthier communities. It shows how to use data effectively to bring about community change, to provide secondary data source references, to illustrate quantitative and qualitative data collection methodologies, and to describe elements of a research and data collection plan.
