This is a  consent form that Dat Drive Detroit uses for its Civiic Users Testing (CUT) Group tests (which include some photo/video components).  It is adapted from the form used for CUTGroup in Chicago, and is scored at 8th/9th grade when I run the text through a reading level checker.
