In 2010, the National Kidney Foundation Michigan (NKFM) was awarded a grant to address health disparities in the African American community in Inkster.   During Phase I of the grant in 2010, NKFM mobilized and engaged diverse organizations and Inkster community residents to form the Inkster Partnership for a Healthier Community (IPHC).  The Phase II project, in 2011 and 2012, builds on the IPHC work of Phase I, and employs a three-pronged approach to reach equitable health outcomes for African Americans.  D3 is the project evaluator for Phase II.
